Thanks for the picture.

A 10MHz amplifier on a bread-board? The poor thing. Probably shock itself to death, oscillating like crazy.

See the data sheet, section about grounding:

"Power Supplies and Grounding

Use a stable dc voltage to power the instrumentation amplifier.

Noise on the supply pins (e.g. high freq noise from a switch-mode power supply - my addition) can adversely affect performance.

Place a 0.1 μF capacitor as close as possible to each supply pin. Because the length of the bypass capacitor leads is critical at high frequency, surface-mount capacitors are recommended.

A ground plane layer helps ... etc"

Better get the proper evaluation board from ADI, or follow their layout in creating your own. The proto-board is probably not going to work.
